Install a single layer of SoundAway Barrier Plus directly over your subfloor, tape the seams, and caulk around the perimeter to create an air-tight seal.  Leave no more than a 1/4" gap around the perimeter then fill the gap with SoundAway Acoustical Caulk.  Apply Lead Seam Tape to close the gaps between sheets of your noise barrier.
